Re-enrollment Process for Current Students
In early Spring, parents/caregivers receive information for re-enrollment, including important dates.
The re-enrollment process consists of three key steps:
Enrollment Process
Complete Parts 1 and 2 of re-enrollment by May 1, 2025, to secure your child’s spot; Part 3 is due before the first day of school.

Complete DC Proof of Residency Form (DCRV) here: English DCRV / Spanish DCRV

Note: The name and address on your documents must match those on the DC residency verification form.
PART 3: Health Documentation
As required by OSSE, submit immunization records. Find more information about DC immunization requirements, guidance, and frequently asked questions here.
Complete the DC Universal Health Form and DC Oral Health form and submit the completed forms here.

Complete the Anaphylaxis Action Plan, Asthma Action Plan, and Seizure Action Plan.
